项目摘要

平成18年度は、得られた質的データより、緩和ケアへ移行するプロセスと影響する要因を特定し質問紙を作成した。質問紙調査を行い、6総合病院の病棟に勤務する看護師204名の回答より看護師が認識する実態を把握した。明らかになった実態は以下のとおりである。(1)緩和ケア導入のプロセスは、病棟での導入が多いが外来もある。(2)緩和医療導入の意思決定の参加度合いは、主治医に比べて患者や家族は低い。(3)緩和医療移行の理由として疼痛コントロールや症状コントロールの必要性がきっかけになることが多く、患者の希望による移行は少ない。(4)緩和医療移行の意思決定への参加度合いは、主治医に比べ患者や家族は低い。(5)緩和医療に影響する要因として、「死を避けたい、治療にかけたいという患者・家族の思い」、「病状に関するインフォームドコンセントの不十分さ」、「主治医の緩和医療に対する考え方」「悪いニュースが伝えられない」が高い。(6)緩和医療移行について、「移行時期が遅い」「移行がうまくいっていない」「患者が主体的に緩和医療を受けていない」「患者と緩和医療のゴールが共有できていない」と感じている。以上の結果より、緩和ケアへの移行プロセスは、身体的状況の悪化がきっかけとなるため主治医の状況判断に拠るところが大きいが、患者・家族の治療への希望や主治医の情報の伝え方に関する課題が影響し、緩和ケア移行における意思決定に患者不在が生じる実態が明らかになった。看護師は緩和ケアの実践において患者とゴールを共有したいと考えており、そのためには緩和医療移行における患者の主体的選択を重要と捉えていた。
