本研究は、自宅での医療的ケアを必要とする子どもをもつ家族の現在抱える問題に対し、解決志向アプローチ(以下、SFA)的手法を用いて、短期間で効果がみられ臨床場面において看護師が実践できる家族インタビューについて検討することを目的とした。本年度は3年計画の1年目であり、研究実施前の準備として次のような検討を実施した。SFA、自宅で医療的ケアを受けている子どもとその家族に関する文献調査では、SFAが臨床場面で実践されている研究報告、自宅で医療的ケアを受けている子どもやその家族に焦点を当てた文献の収集と検討を実施した。その結果、SFAの特徴、効果、医療的ケアを必要とする家族とその家族のニーズと看護師の役割について有効な情報が得られた。医療的ケアを受けている子どもをもつ家族を対象としてSFAが行われた研究報告では、白血病で入院中の子どもの母親のグループを対象としたものはあったが、本研究で対象とする自宅で医療的ケアを必要とする子どもをもつ家族を対象としたものはなく、今後の検討の必要性が示唆された。SFAの手法については、国内および国外のSFA研修会に参加し、SFAの理論、手法について理解を深め、本研究におけるインタビュー方法、内容についての検討を行った。現在、肢体不自由児施設、重症心身障害児施設に通園する子どもとその家族の特徴、通園プログラム、母子の関わりを知り、家族インタビューを実施するためのフィールドの開発、研究参加の依頼を行っているところである。今後は、SFAに精通した臨床家にスーパーバイズを受けながら、家族インタビューを実施し、データの分析を行う予定である。
