项目摘要

看護スタッフが心理教育に参加することで、心理教育に対する認識、日常ケア、患者に対する見方がどのように変化するかを経時的に追跡し、そのプロセスを記述することを目的として以下の調査を行った。精神科救急病棟において新たに患者心理教育を導入した施設を研究協力施設とし、病棟に勤務する看護スタッフ全員を対象に、心理教育導入前および導入半年後にインタビュー調査を実施した。協力が得られたのは24名の看護師で、16名については参加前後2回のインタビューを行った。心理教育導入前には、「内容が分からない不安」「自分自身のスキルに対する不安」「患者・家族への効果を信じられない思い」「スタッフの負担に対する不安」「患者・家族への効果に対する期待」「病棟全体が変化することへの期待」などが聞かれた。導入後は、週1回全4回のプログラムが毎月実施され、6ケ月以上継続されている。スタッフは4回のプログラムのうち、いずれかのテーマを担当しており、プログラムには半年間で1回〜数回参加していた。プログラム参加後の看護師の認識を尋ねたところ、「患者・家族への効果の実感」「患者に対する見方の変化」「患者に対する理解の深まり」「家族に対する理解の深まり」「日常の看護ケアにおける変化」「家族との関係性の変化」「負担はありながらも実施できた達成感」「病棟の変化に対する実感」「自分自身のスキルに対する不安」「患者・家族への効果に対する期待」などが聞かれた。心理教育プログラムの導入にあたっては、スタッフの不安が軽減できるよう事前の研修やミーティングを行うこと、またプログラム導入に伴いスタッフの業務が過度に負担にならないよう、ディスカッションの場や人員配置も含めた組織的なサポートが重要であると考えられた。また、スタッフ自身が患者・家族あるいは第三者からフィードバックを得ることで、プログラムの精錬やスタッフのモチベーションにもつながり、プログラムの継続につながると考えられた。
