【目的】乳幼児健康診査で保険師が継続支援の必要性や支援方針を決定する際の判断材料や判断基準等の根拠を抽出し,その体系化を図る。【方法】本年度は以下について実施した。1.前年度の調査結果の分析前年度の面接調査から得られた,20名の保健師による23事例の継続支援対象事例に対する継続支援方針決定の判断内容から,質的帰納的分析によって判断の構成要素を抽出し,KJ法にその体系化を試みた。【結果】保健師の判断の特徴として次の知見が得られた。1.継続支援方針の決定に関わる判断は,148項目の判断要素を見出され,それらは<事例の実態><継続支援の必要性><継続支援の方法>の3項目に大別された。2.判断に用いられた情報として,<保健師の考え方に枠組み><保健師との関わりの経過><保健師が過去の支援場面で得た情報><保健師が過去の支援場面で判断した内容><保健師が今回の健診場面で得た情報><他の健診従事者が今回の健診場面で判断した内容>の6項目のカテゴリに分類された。3.保健師が継続支援方針を決定する際,情報だけでなく一度判断した要素も次の判断材料として用い,それぞれを「組み合わせる」「積み重ねる」といった方法で判断を深めていた。以上のことから,乳幼児健康診査受診者に対する保健師の継続支援に関する判断は,事例の実態を多様な側面から捉えて評価することと,それを基にして事例に関する様々な将来的予測を立て、継続支援の必要性,支援方法について判断することが瞬時に要求されるため,判断の段階とその判断要素を踏まえた上で,アセスメントや継続支援の目標設定を行うことが重要であることが示唆された。今後の課題として,事例の特性ごとの判断の特徴についても明らかにしていく必要があると考える。
