项目摘要

本研究の目的は,看護サービス財の特性を考慮し,看護サービスを「患者」「看護師」のそれぞれ立場の異なる複数の視点から評価することが必要であると考え,看護サービスの質評価を患者と看護師双方の満足度の視点からの評価方法の検討を行うことにある。平成19年度に実施した質問項目の洗練化を基に200床以上の同意の得られた病院で質問紙調査を行った。質問紙の構成は,前年度の結果をふまえ,満足度をサービスへの"期待"の得点をサービスを"実際"に受けての意見の得点で割り,"満足度"を算出するものを採用した。調査結果は,患者では23項目中10件で満足の状態を示す得点であったが,看護師では,23項目中満足の状態を示す得点であった質問項目は無く,看護師が患者よりも提供している看護サービスを厳しく評価している実態が明らかになった。また,同様の質問項目を用いて行った調査と比較すると,患者の満足が低下している質問項目が23項目中9件あった。これらの質問項目は,患者が評価しやすくベッドサイドに看護師がいることに関する意味合いを持った質問項目であった。これらの満足度が、低下した理由を精査すると,"期待"の得点が上昇しているために算出された"満足度"の得点が結果的に低下していることが判明した。満足度を単に聴く質問項目ではこれらの変化は判別できず,満足度をサービスへの期待の得点をサービスを実際に受けての意見の得点で割り算出する評価方法の有用性が示された。今後の課題としては,今回分析した共分散構造分析のモデルを"期待"の得点を考慮し,層別を行い複数のモデルを作成することで比較するなど満足度全体の構造の変化を分析する必要がある。
