本研究は心身症・神経症患児を持つ父親の役割受容と親役割行動が患児の評価した家族機能と精神的健康に与える影響を明らかにすることを目的とし,小学5年生から中学3年生までの子どもと父親,母親を1組として対象とした。患児群は東北地方の2つの総合病院の小児科看護外来に通院する心身症・神経症患児とその両親14組とし,対照群は2病院が所在する同市にある公立小学校1校の5,6年生225名,公立中学校2校の全学年812名の児童・生徒とその両親,総計1037組とした。その結果,以下のことが明らかになった。1.親の役割受容を患児群と対照群の2群で比較すると,両親ともに差はなかった。親役割行動では差がみられ,両親ともに患児群の「自信」が有意に低く(p<0.05),母親の「受容」は患児群が有意に低かった(p<O.Ol)。2.患児群において、父親の「仕事役割受容」が高いほど子どもの「社会的活動障害」が低かった(r=-0.73,p<0.05)。親役割行動では父親が「受容」しているほど子どもの「自己表明」が高く(r=0.67,p<0.05),「自信」のある親役割行動をしているほど「社会的活動障害」が低く(r=-O.73,p<0.05),「自立促進」しているほど精神的健康が高かった(r=-0.67,p<0.05)。父親の役割受容,親役割行動と子もが評価した家族機能には相関はなかった。3.患児群において,母親の役割受容と子どもの各因子には相関がみられなかった。親役割行動では,母親が「自立促進」しているほど子どもの「自己表明」が高く(r=0.77,p<0.05),「自信」が高いほど「自己受容」が高かった(r=0.74,p<0.05)。また,子どもを「受容」しているほど「自己受容」が低く(r=-0.73,p<0.05),「干渉」しているほど「自己閉鎖性」が高かった(r=0.67,p<0.05)。母親の役割受容,親役割行動と子どもが評価した家族機能には相関はなかった。以上のことから,父親の仕事と家庭における役割受容と親役割行動をアセスメントすることは神経症患児の看護をする際に重要であること,父親が自身の養育に自信を持ち,子どもを受容できるように,父親に対し積極的な介入を進めていくことが親子関係の修復につながる可能性が示唆された。
