项目摘要

平成19年度は、埼玉県内の200床以上の病院に介入研究の協力の依頼を行った。その結果、一施設に協力してもらえることになり、51名の女性看護師がこの研究の対象となった。対象者の背景は、年代20代15名、30代13名、40代17名、50代6名であった。病棟は手術室5名、外科病棟1名、混合病棟16名、整形外科病棟9名、内科病棟13名、脳神経内科病棟7名であった。参加の動機は「リスクマネジメントに興味があった」21名、「上司にすすめられて」29名、複数回答1名であった。平均経験年数13.6±9.1年であった。与薬に関する知識に関する川村の新人看護師に必要な与薬の知識の事前問題61問については、該当しない看護師が全体の約2割以上いる設問が、「mg・g等で指示された小児用の薬剤をmlに換算できる」「5R(正確に実施すべき5項目)を正確に知っている」「同一名称高カロリー輸液(ハイカリック)等の組成の違いを知っている」「同一名称の輸液(ソリタ等)の組成の違いを知っている」「PNツイン溶解を正確にできる」「インスリンの1m1中の単位数を知っている」「冷所保存の薬剤名を把握している」「使用するすべての薬剤の薬効と使用方法を確認後与薬している」「塩酸モルヒネ1アンプルの容量を知っている」「救急カートに準備している注射薬の作用・投与方法を理解している」等15項目であった。研究対象者51名を無作為に、介入群34名、対象群17名に割付を行った。本研究の介入群には、与薬に関する知識を補う内容の「情報」の提供と医療安全の「教育」プログラムを毎月1回を行った。対象者51名全員に1ケ月ごとにインシデント・アクシデント回数と内容を問う、アンケートを行った。現在まだ介入中であり、介入が終了次第統計解析を行う。
