项目摘要

噴門部胃癌は増加しているが、その原因は不明である。粘膜下浸潤胃癌504例をreviewし、領域別(噴門部、中部、下部)に検討したところ、噴門部胃癌は、1.男性に多く、2.喫煙指数が高値で、3.分化型癌が多く、4. Submucosal cystの合併頻度が高く、5.粘膜筋板が肥厚し、6.腸上皮化生が強い、といった特徴が得られた。よって噴門部胃癌では、高度の粘膜remodelingが存在し、上皮のみならず間質にも強い傷害が生じており、間質細胞の遺伝子不安定性・異常があるととが疑われる。以上の背景から噴門部胃発癌は、上皮・間質、特に間質細胞の遺伝子異常が関与し、背景に粘膜remodeliagが影響していることを証明すべく研究を行った。1.臨床病理学的特徴:胃癌間の比較では、上記のような特徴が得られていたが、さらに噴門部胃癌の臨床病理学的特徴を明らかにする為に、非胃癌症例(胃粘膜下腫瘍)をコントロールとして生活歴の比較を行った。結果として胃上部胃癌では、喫煙指数以外に有意にアルコール摂取量が高値であった(P<0.05)。叉、現在までに240例の予後検索をした。5年生存率は90%以上で、領域別差異は見られなかった。2. H. pylori感染:噴門部胃癌におけるH. pylori感染率は63%であった。3.遺伝子異常の検索:噴門部胃癌9例と中部胃癌10例の癌部・非癌部におけるMSI・LOHをNCI推奨マイクロサテライトマーカーと17番染色体マイクロサテライトマーカーを用いて検索した。噴門部胃癌9例中、癌部上皮にてMSIを2例(22%)、LOHを1例(11%)、癌部間質にてMSIを1例(11%)確認した。中部癌10例中、癌部上皮にてMSIを2例(20%)、LOHを2例(20%)、癌部間質にてMSIをO例(o%)確認した。現在までの検討では両群に差異は見られなかった。
