项目摘要

本年度は,昨年度実施された児童を対象とした認知行動療法に基づく抑うつ予防プログラムのフォローアップ調査を行い,プログラムの長期的効果を検討した。本来は米国の子どもを対象として開発された認知行動療法を日本の子どもに適用するにあたり,文化的背景の相違やマニュアルの修正点について米国Oregon Research Instituteの研究者に評価を依頼し,介入の質が担保されていることを確認した。介入群とされた一般小学生150名に予防プログラムを適用し,対照群160名にはアセスメントのみを実施した。介入終了後9ヶ月のフォローアップが可能であった137名(介入群66名,対照群71名)のデータを用いて長期的効果の検討が行われた。介入群では,介入直後に有意に低減していた抑うつ症状がフォローアップ時点においても維持されていることが示された。対照群ではこのような効果は認められず,フォローアップ時点の抑うつ症状は介入前や介入直後よりも高くなっていた臨床的な抑うつ症状を介入前の時点で示していた対象者に限ってみると,フォローアップの時点においても臨床的な抑うつ症状を示す児童の割合は介入群で29%,対照群で50%となっていた。反対に,介入前の時点では臨床的な抑うつ症状を示していなかった対象者の中で,フォローアップにおいて新たに臨床的な抑うつ症状を示していた児童の割合を算出したところ,介入群では3%,対照群では11%が該当していた。以上の結果から,本プログラムは高い抑うつ症状をもともと抱えている児童に対する治療的効果(treatment effect)と,抑うつ症状を抱えていない児童に対する予防的効果(preventive effect)の2つの効果を兼ね備えていることが明らかにされた。
