1500g未満の極低出生体重児の出生数が年々増加し、新生児集中治療室(NICU)の治療とケアは呼吸障害を中心とする長期的な呼吸循環管理へと移行している。呼吸管理中の極低出生体重児にとって、呼吸理学療法は必須のケアであるが、有用性や危険性についてはほとんど検討されていない。本研究の目的は、極低出生体重児に適した気道クリアランス方法の実態を明らかにすることである。全国調査からは、70%以上の看護師が児の安全を維持しながら複数の手技を構成要素とする呼吸理学療法を実施していた。各手技の有効性の評価は困難であるが、発達的予後を含めた評価指標の必要性が示唆された。
