本研究は、介護保険制度が「地域の特性」に応じた制度へと発展するために、2005年改正で新設された「地域密着型サービス」に焦点をあて、事業所の運営状況や利用者の生活実態の現状と課題に迫ろうとするものである。本研究を進めるにあたり、地域密着型サービスの事業所数を把握し、先行研究を収集した。現状として居宅・施設サービスよりも事業所数が伸び悩んでいること、地域によってその整備に偏りがあることがわかった。また、夜間対応型訪問介護、地域密着型介護老人福祉施設入所者生活介護、地域密着型特定施設入所者生活介護の事業所数は非常に少ない状況であった。本研究では主に先行研究や調査もほとんどなく、今後さらにその需要が高まると考えられる(1)夜間対応型訪問介護(全国すべての事業所(2008年3月18日現在))と(2)認知症対応型通所介護(大阪府と愛媛県の事業所すべて(2008年3月18日現在))を対象にアンケート調査を実施した。調査方法は、自記記入式調査票を郵送し同様に郵送で回収した。調査期間は、2008年3月20日から31日までの11日間である。配布数及び回収数は、(1)115票中有効回収票は40票(回収率は34.8%)、(2)182票中有効回収票は35票(全体回収率は19.2%、愛媛30.2%、大阪15.8%)であった。結果をまとめると、両サービスとも在宅介護を続けていく上では必要不可欠であると認識されている、しかし経営状況が悪く労働条件の悪さや人材確保の困難さ、安定的な利用者確保が課題である、通常型サービスとの相違の明確化、利用者やケアマネジャーの認識・理解の低さ、利用者への広報不足等であった。また、夜間対応型訪問介護では事業所数の格差による利用者選択の課題や行政支援の格差(サービス実施にあたっての補助金など)が明確となり、認知症対応型訪問介護においては両地域とも「認知症」に対する利用者・家族・地域・ケアマネジャーの認識不足とその理解促進、スタッフの力量の差と費用面からのスキルアップ研修の不足が課題としてあげられた。
