近年、帝王切開(以下、帝切)率の上昇が世界的傾向として認められており、わが国の帝切率は過去15年間で2倍に増加している。本研究は、増加する帝切分娩の母親に対して、質の高いケアを効率よく提供することを目的としており、そのためにはまずケアの現状を知る必要がある。そこで平成19年度は、わが国で帝切分娩の母親に対して行われている医療処置と看護ケアを把握することを目的に、全国の産科を標榜する病院1765施設の産科部署の看護職代表者1名に自記式質問紙調査を実施した。1.回収結果:回収数は654件で、閉院・廃科58件の返送があったため、実質回収率は38.3%だった。このうち、産科休診中・分娩扱いなし・廃科の111件、未記入の多い2件を除く、有効回答541件を分析対象とした。2.対象施設の概要:分娩件数は平均432件、帝切率は平均22.5%、産科単科は11.8%だった。3.出産前教育:通常の母親学級で帝切に関する情報を必ず提供する施設は43%で、選択的帝切予定の妊婦に対する教育は個別指導が59.5%と多く、帝切予定の妊婦に対するクラス開講は3施設(0.6%)のみだった。4.在院期間と産褥スケジュール:退院日は選択的・緊急帝切ともに産褥8日目が多かった。1日目に初回歩行、3-4日目にシャワー浴開始、5-6日目に沐浴・退院指導、7日目に抜糸(鈎)というパターンが多かった。5.ケア内容:クリニカルパスの使用は86,9%にのぼり、身体的ケア項目は均質化していたが、心理的ケア項目は31%、心理社会的観察項目は23-41%でばらつきがあった。また緊急帝切専用のパスは11%だった。このように、とくに帝切分娩に関する出産前教育の内容と術中・術後の心理的ケア、緊急帝切時の対応を充実させる必要性が示唆されたことから、今後具体的な看護支援の考案に向けた研究を継続する予定である。
