项目摘要

本研究の目的は、有病者歯科診療の安全性の向上のために、歯科衛生士が共有すべき知識と情報提供の方法を検討することである。本年度は、平成18年度のアンケート調査結果をもとに、情報提供が必要と考えられた項目および情報の入手経路についてのアンケート調査票を作成し、前年度に調査協力を依頼した東京都内の歯科診療所1,000件に勤務する歯科衛生士を対象に調査を実施した。調査項目は、10疾患別の「情報が不足している」、と思う関連情報の種類、利用した情報入手経路とその頻度、入手経路別の利用希望および利用しやすさとした。有効回答率は15.7%であり、疾患別不足感のある情報の主成分分析では、すべての変数(緊急時の対応、院内感染予防対策、口腔ケアの方法等)が第1主成分に対して正の負荷量を示した。また、「情報が不足している」と思う関連情報で、8割以上が「とても思う/やや思う」と回答した項目は、精神疾患と血友病の「疾患の医学的基礎知識」、HIV感染症、精神疾患、血友病、てんかん、甲状腺疾患の「緊急時の対応」、血友病の「歯科治療による患者の身体への影響」と「疾患の治療薬と歯科治療との関連」、HIV感染症の「検査データの見方」であった。一方、情報入手経路別の利用希望で7割以上が「ぜひ利用したい/なるべく利用したい」と回答したのは、「専門書」「月刊誌」「一般書」「パンフレット等」「インターネット」であった。これらは「利用しやすい」と回答した割合が5〜7割であったが、「専門書」「月刊誌」「インターネット」は「利用しにくい」との回答も1割以上にみられた。これらのことから、現状では多くの歯科衛生士が有病者歯科関連情報全般に不足感を持っているが、すべての歯科衛生士が「利用しやすい」情報入手経路は存在しないことが示唆された。今後、効果的な情報提供方法についてのさらなる検討が必要と考えられる。
