项目摘要

遊離回盲部移植術後6~39ヶ月の下咽頭・頚部食道癌患者17例を対象に, Videofluorographyによる嚥下機能評価とアンケートによる音声機能評価を行った. 術後6ヶ月以降の嚥下機能は, 吻合口径や再建臓器の走行に起因する咽頭内圧と再建消化管内圧との圧格差や, 再建臓器の蠕動運動などが関与している可能性があり, 遊離回盲部移植法は遊離空腸移植法に比し, この点で有利である可能性が示唆された. また, Videofluorographyの評価結果に基づいた嚥下リハビリテーションを積極的に導入し, 良好な結果を得た. 術後音声機能は88.9%で獲得可能であったものの, 得られた音声機能には症例により差が認められ, 日常生活で積極的に音声機能を使用しているのは30.8%にとどまった. 本術式では, 遊離空腸移植法に比較して気管孔狭窄の発生率が有意に高く, 術後の音声機能の使用に影響を与えている一因となる可能性が考えられた.

17 patients with hypopharyngeal and esophageal carcinoma 6~39 months after free ileocecal transplantation were evaluated by videofluoroscopy. 6 months after operation, the decrease of swallowing function, the cause of anastomosis diameter and reconstruction organ movement, the pressure difference between pharynx and digestive tract, the possibility of reconstruction organ peristaltic movement, and the possibility of free ileocecal transplantation compared with free jejunal transplantation. The results of Videoflurography evaluation are based on positive introduction and good results. 88.9% of the patients had the possibility of using the sound function after surgery, and 30.8% of the patients had the possibility of using the sound function after surgery. Compared with free jejunal transplantation, the incidence of stenosis of the stoma was significantly higher, and the possibility of postoperative use of acoustic function was examined.
