项目摘要

被虐待児における慢性的な身体的虐待の診断根拠となる新旧混在する打撲傷(皮下出血)を客観的に評価するために、測色学的評価と超音波診断装置を用いた皮下出血の深さと厚みとの関連性について検討した。日常生活において小児が転倒するなどして受傷した軽傷な打撲傷では、皮下軟部組織浅層にとどまる軽度〜中等度レベルの皮下出血であれば、測色学的評価に大きな影響を与えないと考えられた。また、紫外線撮影装置を用いた陳旧な打撲傷の観察は、軽度な打撲傷は治癒に伴う皮下出血の吸収が良好であるために紫外線波長に反射しにくいことから明瞭な観察が困難であったが、中等度な打撲傷では辛うじて紫外線による観察が可能であり、同部は黄色調を帯びた部分であることが示唆された。
