项目摘要

今年度はHTLV-1関連ぶどう膜炎や成人T細胞白血病(ATL)症例の受診がなく、新な試料（眼内液と血液）は収集できなかった。このため、前年度までに収集したデータを臨床的に解析した。ATL90例のうち、15例が日和見感染によるサイトメガロウイルス網膜炎を発症し、13例がATL眼内浸潤を発症していた。ATL眼内浸潤症例は男性７例、女性６例で、眼内浸潤発症時年齢は49-70歳（平均59.8歳）であった。ATL発症から眼内浸潤発症までの期間は0日から４年で、2例は眼病変発症がきっかけて全身的にATLであることが判明した。眼内浸潤発症時の全身状態は７例が寛解状態で、４例は化学療法中、眼病変から発見された２例は急性転化の状態であった。片眼性５例、両眼性８例で、ATL眼内浸潤21眼に経過観察中にみられた眼所見は、前房混濁9眼、硝子体混濁14眼、眼底病変15眼であった。眼底病変は網膜内病変、網膜下病変、網膜血管白鞘、脈絡膜病変、視神経乳頭腫脹がみられ、網膜病変は広がりも深さもさまざまであった。眼病変は当初トキソプラズマ、ヘルペスウイルスなどによる感染性ぶどう膜炎や原因不明のぶどう膜炎と診断されて治療され、2週間から6ヶ月(中央値２ヶ月）後にATL眼内浸潤と診断されていた。前房水を採取して前房水中にATL細胞が確認できたのは８眼で、TCR-β遺伝子再構成は検査した３眼中１眼にみられた。眼内液のsIL-2Rは検査した13眼すべてで基準値より高値で、うち9眼は血液のsIL-2Rより高値を示したが、急性転化で血液のsIL-2Rが異常高値であった2眼と明らかな網膜病変のなかった2眼では、眼内液のsIL-2Rは血液より低値であった。眼内液のsIL-2Rは診断の補助として有用と考えられたが、これだけで診断マーカーとすることはできなかった。

This year, HTLV-1 is associated with inflammation and adult T-cell leukemia (ATL), and new samples (intraocular fluid and blood) are collected. This year's edition of the book is published in Chinese. 90 cases of ATL were diagnosed as retinal infection, 15 cases as retinal inflammation, and 13 cases as intraocular infiltration. ATL intraocular infiltration occurred in 7 males and 6 females. The age at onset of intraocular infiltration was 49-70 years (mean 59.8 years). The period of onset of ATL was from 0 days to 4 years, and the onset of ATL was from 2 days to 4 years. The systemic state of ocular infiltration was in remission state in 7 cases, in chemotherapy state in 4 cases, and in acute degeneration state in 2 cases. Ocular filariform 5 cases, ocular filariform 8 cases, ATL intraocular infiltration 21 eyes, anterior chamber opacity 9 eyes, nitrate opacity 14 eyes, ocular fundus disease 15 eyes. Ocular diseases include retinal diseases, infraretinal diseases, retinal vascular white sheath, choroidal diseases, optic papilla swelling, retinal diseases, and deep retinal diseases. The eye disease was diagnosed as intraocular infiltration after 6 months of treatment. ATL cells in aqueous humor were identified in 8 eyes and TCR-β cells were reconstructed in 3 eyes. sIL-2R in intraocular fluid was detected in 13 eyes with baseline values high, in 9 eyes with blood sIL-2R high, in 2 eyes with acute degeneration with blood sIL-2R abnormally high, in 2 eyes with retinal disease with baseline values low, in 2 eyes with intraocular fluid sIL-2R low. Ocular fluid is useful for diagnosis.
