项目摘要

被災地で支援活動をする看護職への精神看護専門看護師のコンサルテーションモデルを考案するために、看護職に対してコンサルテーションを行った経験のある精神看護専門看護師へコンサルテーションを利用する看護職の特徴および実施したコンサルテーションの内容についてのインタビュー調査を行った。研究対象者は、日本看護協会より精神看護専門看護師の認定を受け、所属施設において精神看護専門看護師として活動する8名であった。精神看護専門看護師のコンサルテーションを利用する看護職は、ケア対象者やコンサルテーションを利用する看護職自身、ケアチームについて相談をしていた。看護職は、ケア対象者の状態を問題視し、自分が行ったケアではケア対象者の問題を解決できないと認識していた。また看護職は、ケアチームに対しても自分のケアを非難していると捉えていた。そのため看護職は、ケアを続けていくことに困難を感じ、精神看護専門看護師のコンサルテーションを利用しようと思うようになっていた。精神看護専門看護師は、このように認識している看護職に対して、ケア対象者の状態や行ったケア、ケアチームの言動についての事実確認をするコンサルテーションを行っていた。相談した看護職は、精神看護専門看護師と事実を確認していくことをとおして、看護職自身で自分の捉え方の誤りや別の見方に気づき、それによってケアの意欲を取り戻すことができるようになっていた。今回の結果から、コンサルテーションにおいて精神看護専門看護師は、看護師自身が自分の認知を検討し、新たな意味を見出すためのアプローチを行っていると考えられる。今後は、相談者である看護職が肯定的なケアの意義を見出すことを促す精神看護専門看護師のコンサルテーションスキルを明らかにしていく必要がある。
