要介護度の改善状況を評価するにあたり，新型コロナウイルス感染症感染流行時期とそれ以外の時期を比較検討していけるような研究デザインを再構築すること，また対象者の要介護状態（老化や慢性疾患，認知症）によっては，効果的で十分なサービスを提供し一時的に改善しても状況によってはすぐに悪化したり，または悪化と改善を繰り返したり，改善が見込めない時期に移行したりすることについて，どのように評価すべきか検討を重ねた．その結果，本邦における新型コロナウイルス感染症流行前（概ね2020年1月まで）と，それ以降の時期による違いを検討するために連続横断研究デザインによる評価を行い，感染症拡大時期の影響も踏まえた俯瞰的な評価を行うことにした．次に，性別，年齢，要介護状態（老化や慢性疾患，認知症）やサービスの利用頻度や種類を考慮した決定木分析を適用し，「改善する者」，「一時的に改善する者」，「悪化と改善を繰り返す者」，「改善が見込めない者」にプロファイリングし，その後，各要因についてサブグループ分析を行うことにした．また，複数の交絡因子の調整が困難な場合は，サンプルサイズは減少するが，プロファイリンググループごとに傾向スコアによるマッチングを用いた分析を適用することにした．一方，要介護度認定の有効期間の変更や延長の影響を経年的にどのように評価していくかについては結論が得られなかった．今後は，その影響についても検討し，評価指標には可能な限り普遍性が保たれるようにする．
