全国自治体を対象に、地区担当制の実態と課題を明らかにするために、2017年度の訪問調査をもとに作成した質問紙を用い、全国1671市区町村の統括(的役割)保健師を対象に、郵送による自記式質問紙調査を実施した。有効回答率26.7％、内合併自治体は37.1％を占めていた。活動体制は、地区担当制13.4％、業務担当制25.6％、重層型61％で、重層型の内訳は、地区中心43.7％、業務中心43.7％、その他12.6％であった。活動体制を決めるにあたって影響を与えた要因は、①保健師数、①事業の効率性、③歴代保健師の方針だった。活動体制から生じる業務上の課題で「かなりある」が多かったのは、①業務量の増大、②地域診断が充実しないであった。また、活動体制がもたらす人材育成上の課題は①地域を見て地区活動を組み立てる力の育成、②地区を把握しようとする意識の希薄化、③個から集団・地域へと視点を移すスキルの低下などが挙げられた。地区担当制を取り入れている自治体の37.1％が「時間の使い方」を工夫していた。一方で地区組織は、母子保健推進員68.8％、健康づくり推進員44.3％、食生活改善推進員17.2％の自治体が「組織がない」と答えていた。連携する既存組織でその必要性が高かったのが、①医療機関、②保育所などの子育て関連組織、③民生委員・児童委員であった。地域診断を行っていた自治体は58.1％で、その中で毎年行っているのは32.9％であった。合併自治体と非合併自治体の比較で優位差がみられたのは、以上の結果から、地区担当制の方法は合併の有無より自治体の事情が大きく影響しており、地区担当制に重点を置いている自治体と業務担当制に重点を置いている自治体ではその課題に大きな違いが見られた。活動体制からくる業務上の課題は、将来的には保健師の能力に影響を及ぼし、保健師像を変えてしまうことが予測される。

Regional activities, organization and development of human resources; 2. Regional awareness and development of human resources; 3. Regional integration; 3. Regional perspective; 4. 37.1% of the local governments in the region are responsible for the "time and space". In one area, 68.8% of the local organizations, 44.3% of the local health promotion organizations, and 17.2% of the local health promotion organizations were organized. The necessity of connecting existing organizations is high,(1) medical institutions,(2) child-rearing related organizations,(3) people's livelihood committee members, and children's committee members. Regional diagnosis: 58.1% of the total population, 32.9% of the total population. There is a huge difference in the comparison between merged and non-merged local governments. However, based on the above results, the method of regional responsibility system has a great impact on the affairs of local governments with or without merger. The focus of regional responsibility system is important. If the focus of local governments and business responsibility system is important, there are many problems in local governments. The activity system has been changed, and the future of the business is affected by the ability of the health care provider.
