閉経後ホルモン療法が心血管系疾患(CVD)のリスクを上昇する機序として、エストロゲンに併用する黄体ホルモンである酢酸メドロキシプロゲステロン(MPA)が大きな要因と考えられている。今回の我々の研究から、MPAはエストロゲンの血管内皮機能改善効果を相殺することや、その機序としてMPAが直接一酸化窒素合成酵素(NOS)を抑制し、一酸化窒素(NO)の産生を抑える可能性が明らかになった。一方、MPAはエストロゲンの有害事象の一つである血管炎症促進作用を抑制し、動脈硬化巣(プラーク)を安定化させる可能性も考えられ、MPAにはCVDに対して両面性あることが明らかになった。
