项目摘要

【医療機関のプライバシー】医療空間が狭小である施設では、診察や服薬指導時の情報漏洩は解決しがたい課題である。特に調剤薬局では、医療情報を収集・提供する空間と待合席の空間が一体化したオープンプランオフィスであり、効率的な業務がおこなえるという柔軟性を持つものの、プライバシーを保護し難い面も併せ持つ。薬局の機能的側面からのプライバシー保護と物理的側面からの保護を包括的に検討し、既存の薬局にも導入可能なプライバシー保護対策を検討した。【機能的側面からの対策】オンラインアンケートの結果、調剤薬局の薬剤師の質問に対し患者が当たり障りなく答える理由の一つに会話の漏えいがあること、服薬指導の環境にはプライバシー保護が必要であると感じていることがわかった。会話の漏えい防止については物理的な保護だけでなく薬剤師の工夫(説明、単語、声のトーンなど)が必須であることがわかった。また女性はプライバシー保護をより強く求める傾向があるものの、男女の8割以上が必要性に応じた個別のプライバシー保護が必要であると感じていることから、薬剤師は薬局の特色・責務・患者の立場を考慮して保護指標の優先順位を判断し医療情報の収集・提供に努めることが必要であると考察された。【物理的側面からの対策】医療情報の漏えいを気にせず会話のできる環境は、効果的で安全な薬物治療および医療情報の共有化には欠かせない。そこで薬局の音環境を測定し実態を調査したところ、吸音・遮音・サウンドマスキングによる対策(ABC対策)のうちサウンドマスキングが効果的で、且つ既存の施設にも導入しやすいことがわかった。BGMなどのマスカーおよびスピーカーのマスキング効果について模擬薬局にて検討した結果、不快感を抑えたマスキング方法の有効性が見出された。今後は実用化に向け、医療現場の意見を踏まえた検討が必要である。
