本研究は、在宅医療体制が整備されていない地域に電子メールを用いたメーリングリストによる在宅診療ネットワークを導入し,関係するスタッフの医療・介護知識の向上と,入院日数短縮の有無や再入院回数の低下など医療経済に与える影響を明らかにすることを目的としている.平成23年度は,21年度に開発した,インターネットで閲覧可能な閉鎖型ITネットワーク(チャット形式,以下,ITネット)を共通診療録とし,福井県内でチーム医療による在宅診療を実施している症例について,同意を得て登録を続けた.3年間で19例が登録され,うち6例が死去した.症例の内訳は筋萎縮性側索硬化症(以下,ALS)が6例,アルツハイマー型老年認知症が6例,パーキンソン病が1例(以下,PD),レビー小体型認知症(以下,DLB)が1例,脊髄小脳変性症が1例,嗜銀穎粒性認知症が1例,脳出血後遺症が1例,正常圧水頭症が1例,大腸癌が1例で,ALS2例と認知症の2例を除く15例が全介助状態であった.開発されたITネットは訪問診療を担当する病院4か所,診療所9か所,居宅介護支援事業所19か所,訪問看護ステーション12か所,ディサービス施設11か所,短期入所施設4か所,薬局2か所,歯科医院1か所の協力を得て運用された.このうちALS1例について,チーム医療に参加したスタッフのITネット利用前後における医療・介護知識の変化を検討し,有意差はないものの知識の増加を確認した.また,PDとDLB各1例についても,ITネット導入後の入院回数と在院日数の変化を検討し,入院回数は変化がないものの,在院日数は短縮される可能性が示唆された.開発されたITネットは福井大学地域医療推進講座の研究事業に引き継がれ,長期間の研究継続が可能となった.

There were 6 cases of endomuscular atrophic lateral sclerosis (ALS), 6 cases of senile cognitive disorder, 1 case of senile cognitive disorder (PD), 1 case of microsomy cognitive disorder (DLB), 1 case of spinal microsomy cognitive disorder, 1 case of argyrophilic cognitive disorder, 1 case of bleeding sequelae, 1 case of normal hypertension syndrome, 1 case of colorectal cancer, 2 cases of cognitive disorder and 15 cases of complete mediated state. The development of IT facilities includes visiting clinics 4, clinics 9, residential care support facilities 19, visiting nursing clinics 12, visiting clinics 11, short-term clinics 4, clinics 2, dental hospitals 1, and collaborative efforts. This article discusses the transformation of medical and nursing knowledge before and after the utilization of medical and nursing knowledge, and confirms the increase of medical and nursing knowledge. 1 case of PD and 1 case of DLB respectively. The possibility of hospital stay shortening after the introduction of IT was discussed. It is the first time for IT researchers to develop a research career in Fukui University Regional Medical Promotion Seminar, and long-term research is possible.
