项目摘要

オープンステント内挿術268例について調査し、脊髄虚血発症9例とその他259例の比較検討から、術前糖尿病、慢性解離・動脈硬化性動脈瘤、大動脈粥状硬化、術前腹部大動脈瘤手術、Th9以下に内挿した例、左鎖骨下潅流なし、術後低血圧の症例に有意に脊髄障害の発生が多いことが分かったことを踏まえ、術前三次元CT検査で弓部大動脈病変の範囲、アダムキーヴィッツ動脈の部位を同定し術中脊髄誘発モニターを体外循環前、体外循環直後、冷却中34度、冷却中32度、冷却中30度、冷却中28度、下半身循環停止時、ステントグラフト留置後　10分毎、末梢吻合後、順行性体循環再開後、復温中体外循環中、復温後体外循環中、体外循環停止後、閉胸直前、手術終了時の時点に測定し、同時に体血圧と心拍出量を計測した。その結果、手術中の脊髄誘発電位変化は体血圧と大きく関係していることが判明してきた。特に術前リスク因子としてあげられる、糖尿病、慢性解離・動脈硬化性動脈瘤、大動脈粥状硬化、術前腹部大動脈瘤手術、Th9以下に内挿した例に顕著に表れ、術前脊髄虚血ハイリスク例においては周術期の血圧管理の重要性が示唆された。
