本研究は、進行食道癌に対する化学放射線療法を施行した症例について、MRIの拡散強調画像から見かけの拡散定数（ADC値）を測定することで、治療開始から早期の段階で最終的な治療効果が予測できるかどうか、及び 治療開始からどの時期にMRIを撮像するのが望ましいか、について検討することを目的とした。具体的には、治療開始前及び治療期間中数回、拡散強調画像を含むMRIを撮像し、その画像からADC値を測定、治療開始前と治療期間中の値の相違や値の変化の仕方と実際の治療効果を比較し、統計解析を行って相関関係を求めるという手法で行った。本年度は研究開始2年目として、必要なデータの収集を継続した。食道癌のように縦隔に存在する病変は、部位の問題からMRI、特に拡散強調画像を得る際に、呼吸性異動や心拍動によるアーチファクトや像のゆがみが問題になりやすいが、これまでの経験から撮像方法の工夫などを行い、食道癌でも進行癌のような比較的大きな病変については十分実測に耐える画像を得られるように至り、全例でADC値の測定には成功した。また、ADC値を測定する際にフリーハンドで関心領域（ROI）を設けて測定する方法で行ったが、ある程度の経験を積めばフリーハンドでのROI設定でも十分再現性のある値を測定できることが判明した。また、全例において化学放射線治療に伴いADC値が変化することを確認した。本年度末での研究代表者の遠隔地への異動が決定し、研究の継続が事実上不可能となったため本年度にて打ち切りとした。症例の集積と観察期間がまだが十分でないために、具体的な治療効果とADC値の関係性についての検討や、最適なMRI撮像時期の検討には至らなかったが、得られたデータ等については無名化の上施設に保管し、今後別の研究者が同様の研究を行う際の参考になるようにした。

All cases were successful in ADC measurement. The method for determining the ADC value and the ROI setting is very reproducible. All cases of chemoradiotherapy are confirmed by ADC. At the end of the year, the research representative's long-distance changes have been decided, and the research has been completed. The collection and observation period of the cases are divided into ten parts, the specific treatment effect and the relationship between ADC value are discussed, the optimal MRI imaging period is discussed, and the nameless upper construction is kept, and the reference for future research of the same kind is carried out.
