平成25年度は計画通り、外部照射加速部分乳房照射(APBI;Accelerated Partial Breast Irradiation)の各患者の呼吸性移動を加味した線量分布を計算する手法の検討を行った。心臓線量の評価も行うためにAPBI治療実施患者のうち左乳房6症例を対象とし、治療前に取得したkV-X線透視画像データから得た乳房内留置クリップの時間-位置変動情報を基に、呼吸性移動の振幅の中央を3次元座標(0, 0, 0)(座標0mmは-0.5㎜以上0.5mm未満の範囲を示す)として移動位置座標を1mm間隔に分別しクリップ重心の各移動位置の存在確率を算出した。存在確率は6症例いずれも、各方向の移動量-0.5㎜以上0.5mm未満は21％以上、-1.5㎜以上1.5㎜未満は82％以上であり、照射中の8割超の時間においてクリップ重心位置は振幅中央から各方向±1.5㎜の範囲内に存在することが推測された。さらに、呼吸性移動を加味していない元プランのビーム中心が座標(0, 0, 0)の位置であると仮定して、呼吸性移動量に対応して各軸1㎜間隔で治療ビーム中心位置を変移させ各位置での線量分布を計算し、各線量分布を算出した存在確率に従って重み付けして合算し、呼吸性移動を仮想的に加味した線量分布を作成した。線量分布計算には治療計画装置iPlan、分布の重み付け・合算には放射線治療計画支援ソフトウェアMIM Maestroを用いた。ターゲットやリスク臓器(心臓、左右肺、左右乳房)の線量体積ヒストグラムの数値指標を評価した結果、元プランに対する呼吸性移動を加味したプランの6症例の各臓器の数値指標の変化量は±0.6パーセントポイント以内、6症例の平均値は±0.2パーセントポイント以内であり、呼吸性移動によるターゲットやリスク臓器の線量変動は小さく、臨床的な影響は小さいことが治療計画上で示唆された。
