本研究は、高齢者施設において、日中ほとんどの時間を座位姿勢で過ごす高齢者を対象に、「下肢浮腫軽減に向けたケアプログラム」を開発し、その有用性を検証することを目的とする。平成25年度は、高齢者施設に勤務する看護職員を対象に「高齢者施設における下肢浮腫に対するケアの実態調査」を実施し、高齢者施設における下肢浮腫に対するケアの課題を見出すことを目指し、以下の2点を中心に研究をすすめた。1．下肢浮腫に対するケアに関する文献検討国内外の下肢浮腫に対するケアおよび高齢者施設における下肢浮腫に対するケアについての先行研究の確認を行い、下肢浮腫に対するケアの実態調査に用いる調査票の内容・項目を検討した。文献検討の結果、下肢浮腫やそのケアに関する先行研究は、褥婦やリンパ浮腫のある患者を対象とした報告が多く、高齢者を対象とした研究はマッサージ等を取り入れた事例研究が数例あるのみであり、とくに高齢者施設の下肢浮腫のケアの実態は明らかになっていないことを確認した。2．高齢者施設における下肢浮腫に対するケアの実態調査介護老人保健施設において、看護職が下肢浮腫のある高齢者に行っているケアの実態を明らかにし、施設入所高齢者の下肢浮腫軽減に向けた効果的なケアを検討することを目的に、郵送による無記名自記式質問紙調査を実施した。研究対象は、近畿圏内にある介護老人保健施設に勤務する看護職とし、WAM-NETに登録されている近畿圏内の介護老人保健施設（556件）に質問紙を郵送した。主な調査内容は、下肢浮腫のある高齢者の有無、下肢浮腫のある高齢者に実施しているケアの内容と方法や頻度、下肢浮腫のある高齢者に対してケアを実施した理由等とした。3月末で退職のため、本研究課題はやむを得ず廃止することになったが、調査結果については今後分析を進め、高齢者施設における下肢浮腫に対するケアの実態について明らかにする予定である。

This study aims to explore and demonstrate the usefulness of the "lower extremity edema reduction" program, which is designed for high altitude patients, and the time and position of the patient. In 2015, the project of "Investigation on the Status of Lower Limb Edema in Higher Education Facilities" was carried out by nursing staff in higher education facilities, and the following two points were discussed. 1. The literature review on lower extremity edema related to lower extremity edema reviewed the confirmation of previous studies at home and abroad on lower extremity edema related to lower extremity edema and lower extremity edema related to lower extremity edema in patients with hypertension, and reviewed the contents and items of the survey questionnaire used in the actual survey on lower extremity edema related to lower extremity edema. The results of literature review, lower extremity edema, and related studies, including several case studies, were reported on patients with lower extremity edema. 2. A survey on the status of lower limb edema in elderly care facilities, nursing care facilities, and elderly care facilities was conducted. The main investigation contents include the presence or absence of edema in lower limbs, the content and frequency of edema in lower limbs, and the reasons for its implementation. At the end of March, this research topic has been studied for the purpose of further analysis, implementation of high altitude patients, lower limb edema, and predetermined status.
