本研究では、高齢患者が、白血病という診断を受け、化学療法に臨む過程において、疾患や療養生活についてどのような認識・姿勢・思いで臨んでいるのかについて具体的な理論の構築を目的とした。65歳以上の初発の白血病の高齢患者15名に面接を実施し、分析方法は、修正版グラウンデッド・セオリー・アプローチを使用した。分析の結果、〈状況認識〉〈状況反応〉〈医療者との関係〉〈周囲との関係〉の4つのカテゴリーを抽出した。告知の際、高齢患者は、白血病を不治・希有、治療をを過酷と捉えた白血病・治療への通念や白血病と他の病気を区別し、白血病の大変さを認識する白血病と他の病気の区別化を行うといった状況認識を行っていた。この段階の患者の中には、ここまで生きたからあきらめるといった思いを抱いた者もいた。しかし、患者は、治療開始前には、覚悟を決める、医師に任せるしかないと気持ちの切り替えを行っていた。気持ちの切り替え時には、医師による治療の勧めや家族が化学療法の進歩を伝えるといった働きかけがあった。治療開始後、患者は治療専念行動を行い、自分は治る、医師を信じようという闘病意欲の強化は最も行われていた。闘病意欲の強化は、副作用が強い時や見通しがたたないという治療へ負担感を実感する時にも行われていた。一方で、患者は、治療専念行動と同時に今を生きるという試みも行っていた。この試みは寛解期を迎えてからも行われ、患者は、今を生きるという試みと共に安心ではないをいう気持ちを抱いており、「よくなったね」という他意のない励ましに違和感を覚える患者もいた。以上の過程には医療者及び周囲の関わりが関連し、特に医師との信頼関係は患者が治療を続けていくうえで重要なものであり、また、家族からの頼られる立場といった存在価値創出関係も患者の闘病意欲を支えていた。同病者との交流も、大切な情報であり、その戦う姿が患者の闘病意欲を支えていた。
