本研究の目的は、母子健康手帳交付(妊娠の届出)の実態を明らかにすること、および子育て支援の観点から保健師による母子健康手帳交付の特徴を明確にすることである。本年は母子健康手帳交付に関する情報収集と整理、アンケート調査を行った。情報収集は、文献やWEB等により、交付時の妊婦への調査、周産期からの虐待予防に関することや、住民への母子健康手帳交付の周知方法・内容等を把握した。アンケート調査は、51市町村を対象に自記式郵送法にて配布し、33市町村(64.7%)より回答を得た。交付体制について、交付場所が1箇所のみの市町村は17(51.5%)で、他市町村は複数箇所であった。また電子申請を併用している市町村もみられた。交付担当者が必ず常勤保健師である市町村は8(24.2%)、主に常勤保健師である市町村は、10(30.3%)であった。通常のおおよその交付時間は、全市町村の平均が12分で、最多回答は10分、9市町村であった。交付時にはほとんどの市町村において、何らかの指導や情報提供を行っていた。交付をきっかけとした要支援者の把握は、実際に交付した担当者の判断による市町村が27、指標による把握が8市町村(重複回答有)であった。要支援者に対する妊娠期の支援担当者は、母子保健担当保健師が最も多く、次いで地区担当保健師であり、その支援方法は担当者が個別に必要に応じて支援するという回答が約半数を占めた。また、現行の交付体制で十分であると回答した市町村は17(51.5%)、十分でないと回答したのは10(30.3%)であった。今後はアンケート調査の分析をさらにすすめることと、保健師による交付の特徴の明確化に取り組む予定である。

The purpose of this study was to clarify the status of maternal and child health account delivery (pregnancy delivery) and to identify the characteristics of maternal and child health account delivery for child care providers. This year, information collection, collation and survey related to the delivery of maternal and child health accounts were conducted. In the future, the analysis of the survey will be carried out, and the characteristics of the delivery will be clarified and determined by the health care provider.
