1. 研究目的 : 掻破行為もしくは掻破痕を有する寝たきり高齢者の皮膚状態と掻破行為の実態から、その類型化を試みるとともに、掻痒の原因から必要なケアを案出することを目的とする。2. 研究方法 : 新潟県下の特別養護老人ホーム2施設で、掻破行為もしくは掻破痕があり、かつ許諾を得られた入所者に対して、左下腿部と掻破部位の皮膚状態の観察評価(油分・水分・PHコンビネーションタイプ[○!R]CK社製での実測を含む)、施設職員への聞き取りまたは質問紙による対応の実態調査、および既存記録によるデータ収集を行った。観察評価は平成21年1月に実施した。3. 結果および考察 : <対象者の属性>上記の条件にあう入所者は、ユニット型のA施設13名、多床室型のB施設17名の入所者であった。A施設は女9名、男4名、平均年齢92.15±3.84歳、平均入所期間は375.38±141.23日。B施設は女11名、男6名、平均年齢89.12±6.44歳、平均入所期間は1544.71±67.95日。<皮膚状態と掻破行為の結果>観察時の環境は、A施設は平均室温23.08±1.19℃、平均湿度42.09±4.61%。B施設は平均室温23.67±1.60℃、平均湿度30.87±5.33%であった。左下腿部の皮膚の角層水分含有量の平均値は、A施設34.8±10.5、B施設38.8±9.8。油分の平均値DA施設1±0.59、B施設1±0.48。PHの平均値はA施設5.5±0.6、B施設5.3±0.9。掻破がある部位を大きく頭部・上肢・下肢・体幹に分類すると、最も多い部位は体幹であり、中でもオムツや下着の当たる部位(腰・臀部・腹部)が最も多かった。このことから、オムツ交換などの排泄ケアが掻破行為の誘因の一つと考えられ、排泄ケアの検討の必要性が示唆された。更に分析を進めており、掻破部位や痕の違いから皮膚状態の類型化とその原因やケアについて明らかにする。
