项目摘要

これまで現地調査を元に結果を論文や学会で発表してきた。農園・農村は都市部に比べ、BMIの低水準や貧血が多くみられ、母子の健康に関する潜在能力が乏しい。とりわけ農園では妊娠末期でBMIが18という痩せた妊婦もおり、母子の生命のリスクが高い。農園の母子の健康問題の背景には医療・福祉制度の他に、ひとの生存に直接的に関わる基礎的潜在能力としての本質的自由、すなわち生活基盤(生活条件や所得水準)、教育基盤(基礎教育の享受とその水準)、健康基盤(栄養状態や健康管理能力)があげられる。さらに上記に関連する手段として、制度やシステムを享受する政治的自由(農園の移民の場合は市民権の取得)、経済的・社会的自由(雇用の機会や社会的機会へのアクセスと参加)、安全と保護の保障(公的制度及び農園独自の福祉制度や雇用に関する保障、それへのアクセスの仕方)において、ひとの機能の拡大(潜在能力の拡大)が求められる。また農園・農村では社会、文化、伝統的規範の影響の中で、女性のエンタイトルメント(権原)が脆弱で妊婦の意思決定能力が乏しい。そのため自らのwell-beingに関心をもつ行為主体者への役割の転換、ヘルスの履行者としての主体的な人間としてのエンタイトルメントの強化が求められる。潜在能力向上に役立つ機能的な観点からは、well-being freedom(福祉的自由)の視点がとくに農園には重要である。それは一律的な基礎的潜在能力の充足だけでなく、ひとがよい生き方やありようを達成するための自由及び機会の均等、すなわち妊婦の妊娠-出産-育児のプロセスにおいて、政治経済、社会文化的アクセスの機会や制度の活用、保護の保障、女性のエンタイトルメントの向上など、母子が望ましい健康を保つうえで必要なひとの機能を達成する条件を整えることが必要である(日本母性衛生学会誌へ現在投稿中)。
