本研究の目的は(1)A大学病院の非糖尿病専門外来における未介入の糖尿病予備群の現状把握、(2)予備群対象者のQOL及び心理・社会的側面に焦点を当てた集団及び個別予防教育プログラムを構築し、試験的に実施・評価し、糖尿病発症予防に資することである。今年度は、目的(1)について2007年4月から2008年3月の同病院の糖尿病非専門診療科における糖尿病予備群調査を実施し、これまでに次のことを明らかにした。(1) 18歳以上でHbA_<1c>値が5.2%以上6.1%未満、または随時血糖値が140mg/dl以上200mg/dl未満の糖尿病予備群の可能性が否定できない3,734件のうち、糖尿病予備群(予備群)は2,638件(71%)、DM群は564件(15%) 2,638人であった。予備群は治療上副腎皮質ホルモンの使用がない非ステロイド予備群(2,010件(54%))と副腎皮質ホルモンを使用しているステロイドDM予備群(628件(17%))に分類された。(2) 循環器内科、リウマチ・膠原病内科、神経内科、呼吸器内科等、副腎皮質ホルモンを比較的大量に使用する診療科において予備群が多かった。(3) 40歳以上で糖尿病の可能性が否定できない者(HbA_<1c>値が5.6%以上6.1%未満)は20.5%と厚生労働省の平成19年国民健康・栄養調査の推計16.9%を上回る結果となった。(4) 18歳以上40歳以下の若い年齢層からの糖尿病予備群の存在が明らかとなった。これらの成果については国際学会で発表し、現在、雑誌論文に投稿中である。

(2) the department of environmental medicine, the department of primary disease, the department of psychiatric medicine, the department of respiratory medicine, etc., and the department of accessory dermatology use a large number of people in the department of environmental medicine. (3) the probability of diabetes over 40 years old negates the possibility of diabetes (over 6.1% in HbA_<1c>). 20.5% in the province of Pingcheng National Health Health in 19,16.9% of the results show that the results are negative. (4) if the age is over 18 years old and below 40 years old, there is a clear diagnosis of diabetes mellitus in the population. The results will be published in the International Society of International Studies table, now, and in the contribution of the journal article.
