项目摘要

平成24年度は下記二つの作業を行った。(1)QOL概念に関し、医療の領域の様々な説明を整理した上で、異なる説明の前提にある「よき生」についての考え方を分析し、いかなる考え方を採用するのが妥当なのかを検討すること、(2)療養行動支援のあり方に関するこれまでの研究成果をまとめ、支援のあり方に対し具体的な提言を行うことである。(1)の作業では、次のことを明らかにした。第一に、医療の領域ではQOLの理解をめぐって、(a)「誰が特定の患者のQOLを判断するのか」、(b)「判断のための社会に共通の指標を設定することは可能か否か」という二つの点をめぐって相異なる見解があること、第二に、(b)をめぐる相違は、「"よりよい生活"についての価値判断は多様でまったく共有できないと見るのか、少なくとも最低限において共有可能だと見るのか」をめぐる相違に求められるが、共有可能と見るのが妥当だということ、第三に、(a)については、「人の生活は一連の基本的な活動から成っており、その活動に必須の条件が整えられ選択して行うことのできる活動の範囲がある程度維持されている場合、生活は"よりよい"と判断され得る」という考え方に基づくなら、QOLは、患者だけでなく医療従事者の参加によってより正確な判断が可能になるということである。(2)の作業では、療養行動支援の手法として慢性疾患医療の領域で提案されている新たなアプローチを「QOLの維持」および「患者中心の医療」の観点から評価を行った。それを通じて明らかにしたことは、新たな三つのアプローチは、まず、身体活動以外の患者の状況への配慮および患者の個別性への配慮を行うものだという点で「QOLの維持」に優れており、そして次に、双方向のコミュニケーションによって支援が進められる点で、「患者中心の医療」として優れていること、従って、従来のアプローチよりも新たなアプローチによって支援を行うことの方が倫理的に望ましいことである。
