本研究は、近年東北地方の多くの地域で展開されている公立病院再編に注目し、それが住民生活に与えているインパクトを明らかにすることを目的としている。今年度は、①農山村の医療行動に関する論文の公表、②農山村の地域医療再生に関する分担執筆、③博士学位請求論文の執筆を行った。これらの概要は、次にまとめられる。①では、2012年に山形県小国町において実施した調査の結果を分析した。山形県小国町は、都市的地域からの隔絶性が高く、医療行動には過大な移動負担が伴っている。そのため、医療行動は生活問題につながりやすいと考えられ、通院、入院行動調査を行った。しかし、その結果、当該地域の医療行動には、別居家族などによる支援が柔軟に取り入れられており、医療行動に条件不利性の強い農山村では、医療行動で起り得る生活問題を回避しようとする戦略的側面があるこζが明らかとなった。②では、農山村の医療問題の歴史的変遷を踏まえ、医療問題の今日的特徴、生活問題としての特徴、地域医療再生の方途について議論した。その結果、今日は医療体制の整備が都市部に比して立ち遅れているという従来の問題から、ようやく構築した医療体制の維持が困難化しているという局面をむかえており、既存医療体制の縮小・撤退は、地域への定住意志をも揺るがす問題を引き起こしていることを指摘した。③では、次の2つを課題とした。第1は、医療体制の縮小・撤退による住民生活への影響の解明であり、第2は、医療体制を維持するガバナンス構造の考察である。前者について、公立病院改革の一環として公立病院を無床化した地域における調査によると、当該地域では医療機会が得られなかった病状の不安定な患者の在宅療養が発生し、家族介助者に過大な負担が生じていることが明らかとなった。後者について、公立病院改革を病床削減ではなく病院の経営改革で対応した地域における調査によると、公立病院と自治体が地域づくりの一環としてともに改革に取り組む関係の構築が重要であることが明らかとなった。

The former is a part of the reform of public hospitals, and the public hospitals are free of beds. When medical opportunities are available in the regions, the unstable patients are treated at home, and the burden of family mediators is too great. In the latter case, the reform of public hospitals should reduce the number of beds, and the reform of public hospitals should be carried out in accordance with the regional conditions.
