We evaluated the changes in the levels of serum anticholinergic activity (SAA) during the pre- and postoperative periods, and investigated the relationship between the levels of SAA and the occurrence of postoperative delirium. Patients undergoing elective esophagectomy or gastrectomy under general anesthesia were prospectively studied. The levels of SAA were measured immediately before and after the surgery, then categorized into three groups, (+), () and (-) according to the results.Postoperative delirium was identified in 41.2% of the 34 patients enrolled in this study. The preoperatively SAA (+) patients who remained SAA (+) or SAA () after surgery were more likely to develop postoperative delirium. It was speculated that the cholinergic system failed to compensate for the elevation of the SAA level in patients who developed delirium.In conclusion, the perioperative transition of the SAA levels may be associated with the occurrence of postoperative delirium.

我们评估了术前和术后期间血清抗胆碱能活性（SAA）水平的变化，并研究了SAA水平与术后谵妄发生之间的关系。前瞻性研究了在全身麻醉下接受择期食管切除术或胃切除术的患者。分别于术前、术后即刻测定SAA水平，根据结果分为（+）、（）、（-）三组，本组34例患者术后谵妄发生率为41.2%。术前SAA（+）的患者术后SAA（+）或SAA（）的患者更容易发生术后谵妄。因此推测，术中发生谵妄时，胆碱能系统不能代偿SAA水平的升高，SAA水平的变化可能与术后谵妄的发生有关。
