项目摘要

高齢者介護施設におけるサービスの質向上を目的に，①高齢者介護施設における職員の作業疲労評価，②高齢者介護施設における感染症対策に関する調査を実施した．【介護施設におけるWLBコンサルティングの導入と評価】千葉県内の介護施設職員を対象にワーク・ライフバランスコンサルティングを1年間1パッケージで導入した．介入の評価を，生化学的指標および活動量計，フリッカー検査，質問紙により測定した．結果，介入前後での唾液中コルチゾールの有意な差が認められ，介入による疲労軽減が示唆された。また活動量計の測定結果からは，部署ごとの活動量の差を比較検討した．【介護業務の疲労評価による改善提案】神奈川県内の介護施設職員を対象とし介護作業の疲労を測定した．特に食事介助・排泄介助・入浴介助について，その作業前後のフリッカー値測定，勤務時間中の活動量測定，問診により評価した．結果，作業前後で疲労の蓄積が示唆された．また，前述の三大介護における活動量以上に夜勤帯におけるコール対応による活動量が連続して高く，こちらについては以後も検証が必要である．【介護施設における感染症予防策調査】全国の特別養護老人ホームを対象に，感染管理についての質問紙調査を実施した．研究は縦断研究で，H25年度・H26年度に実施した調査をうけた追跡調査である．H25・H26年度の調査からは，感染症発生は施設内における研修や感染症の情報周知といった感染対策の実施有無よりも，看護師能力や施設属性に左右されることが推測される結果となった．この結果は予想外であり，より詳細に調査すべくH26年度新たに施設属性および看護師属性の項目を精査し追跡調査を行った．以後，施設沿革から看護師養成の状況なども一因として検討する予定である．また現場との乖離は質問紙調査の限界であり，今後ヒアリング調査・フィールドワークの実施も予定している．
