项目摘要

本研究では，レスパイトケアの在宅療養支援モデルの検討を通して，在宅療養者の公平社会の実現を目指した．研究プロセスは，フェーズⅠでは在宅療養におけるレスパイトケアの問題の把握と分析，フェーズⅡでは支援モデルの叩き台作成，フェーズⅢ支援モデルの試行と改良，フェーズⅣ支援モデルの普及・採用とカスタマイズであるが，本研究では，フェーズⅠまでを目標とした．2020年度は，1年目から2年目に実施したフェーズIの在宅療養支援におけるレスパイトケアの問題の把握および，問題の分析を行うための全国調査から得られた，おもにドナ・ベディアンの医療の質評価モデル（Donabedian, 1980）の枠組みのストラクチャー要因が影響を及ぼしていること，プロセスやアウトカムが影響を及ぼさなかった現状から，日本におけるレスパイトケアは形成期であることが考えられた知見をもとに最終年度の研究計画を修正した．修正した最終年度の研究計画は，レスパイトケアを成熟させ一般化していくために，cross-sectional studyでは因果関係の検討が困難という限界を踏まえ，さらに大規模な調査による現状把握のために，国民健康保険介入者の基本台帳，特定健診，医療レセプト，医療摘要，介護給付の複数年にわたる各種ビッグデータ（KDB）を用いた既存データ解析による在宅療養支援におけるレスパイトケアの問題の分析を行った．また，KDBでは網羅することが困難である家族介護者の問題の分析については，在宅ホスピス・緩和ケアを利用して亡くなったがん患者の家族介護者を対象に，主要な介護負担の同定と，主要な介護負担に関連する患者と家族の要因検討を介護保険制度に着目して行った．本研究に係る一連の研究成果として本年度は、2本の国内外の学術論文発表を行い，現在3本の学術論文を国際学術誌に，1本の学術論文を国内学術誌に投稿中である．
