项目摘要

高齢者におけるフレイル予防は，超高齢社会の喫緊の課題である．フレイルを示す老年症候群には尿失禁や便秘などの排泄症状が挙げられる．排泄症状の中でも夜間頻尿や過活動膀胱，切迫性尿失禁と転倒・転倒骨折は関連があると報告されており，転倒・転倒骨折はフレイル・サルコペニアと密接に関係していると考えられている．本研究は，フレイルと排泄症状，転倒や転倒に至る前段階の転倒インシデントとの関連を明らかにし，排泄に関連する転倒予防のために健康講話を実施して各症状改善の効果検証を行うこと，また気候帯の異なる地域での差異を検証することを目的としている．昨年度同様に，今年度も前半までは新型コロナウィルスの変異株の感染拡大があったため，各地域の通いの場等で活動に参加している地域在住高齢者を対象とした体力測定や質問票調査，排泄関連の転倒の予防のための健康講話は，感染状況をみながら実施することとなった．日本語版フレイル基準によるフレイル判定で，健常群とフレイル・プレフレイル群の2群に分けると，フレイル・プレフレイル群が5割であった．フレイル・プレフレイル群では，サルコペニア有症率は3割，2回以上の夜間排尿回数は5割超，過活動膀胱は3割超，尿失禁は6割超の割合で有していた．過去1年間の転倒経験は3割超で，その半数が過去6か月に夜中の転倒インシデントおよび転倒を経験していた．地域在住高齢者を対象に，体力測定や質問票調査，健康講話を行い，効果検証を実施する研究計画は次年度へ継続とする．
