项目摘要

本研究の目的は、公立病院の需要関数の推計を通じて、（1）公立病院が直面している経営赤字問題を、理論的・実証的に分析すること、及び、（2）将来的に実施されると予想される医療費抑制政策の効果を、シミュレーションによって予測することである。経営赤字を分析する際には、供給側の要因と需要側の要因の両方を考慮することが望ましいため、本研究では、供給側と需要側の両方を考慮した市場均衡モデルの構造推定によって、公立病院の需要関数を求める。本研究で用いる需要関数の推定法（Imai et al., 2022）は、まだ開発段階にあり、2022年度時点では、残された課題のひとつに、需要関数の全ての変数の一致推定量を求めることがあった。このため、研究開始から2年目の2022年度も、セミナーでの研究報告と共同研究者との議論を通じて、操作変数を用いずに需要関数の全ての変数の一致推定量を求める手法の開発に取り組んだ。具体的には、北海道大学経済学研究院の主催の北大近経セミナーにて、開発中の手法について報告し、その後、共同研究者と対面で手法の課題について対面で議論した。その結果、操作変数を用いずに需要関数の全ての変数の一致推定量を求める方法をひとつ提案することができた。さらに手法の開発と並行して、利用可能な国の統計データについてのみを用いてデータセットを構築し、病院経営の効率性について定量的に分析した。具体的には、確率的フロンティア分析と包絡線分析法を用いて、公立病院の経営効率性を計測した。そして、分析結果をthe 31st Asia Economic Symposium，及び，the 17th International Convention of the East Asian Economic Associationにて報告した。

The objectives of this study are: (1) to estimate the needs of public hospitals;(2) to estimate the effects of future implementation of medical cost control policies; and (3) to estimate the needs of public hospitals.
