矯正歯科治療後の咬合の安定・保持には、矯正治療後の歯列弓形態や咬合に適応した頬舌側に負荷される力の平衡性が成立しなければならず、これらの計測値を統合して検索する必要がある。本研究は、超音波診断装置を用い、簡便で被曝のない頬舌圧と舌運動の同時記録法を開発し、安静時や嚥下時の舌位、舌運動と頬舌圧を同時に観察、測定し、咬合安定に寄与する軟組織の各種因子の数値データを算出、解析することで、矯正治療後の長期安定のために歯列形態が保持すべき条件を解明することを目的とする。今年度はまず、舌運動－頬舌圧同時記録法を用いた検査システムの整備に着手した。頭部と超音波プローブの位置関係を保つ頭部固定装置の試作や、舌運動動画データと頬舌圧計測値が同一画面に表示され、計測結果を被験者毎に自動保存するプログラムの構築など、本システムの運用に必要なハード及びソフト面での準備を行った。今後は被験者のリクルートとデータの収集：正常咬合者及び矯正歯科治療前後の患者を被験者として、舌運動－頬舌圧同時記録法による検査、記録を行う。被験者のリクルートとデータの取得にあたっては東京医科歯科大学歯学部倫理委員会の承諾を得て行う。被験者は矯正治療前後の患者および中学生以上の正常咬合を有する男女各10名、候補として東京医科歯科大学歯学部附属病院矯正歯科外来で矯正治療を行い、自然保定に移行した患者、正常咬合を有する当分野医局員、本学歯学部附属病院研修医、学生で、本研究の内容を理解した上で本研究への参加に同意した者とする。
