知的障害者は口腔の健康に関わる様々な問題が指摘され、口腔の健康を維持するためには障害特性に伴う日常生活行動の阻害要因、歯磨き技術の習得度、歯科保健指導内容に対する理解度や意思疎通方法等に特別な支援を要すると考えられる。そこで本研究は、特別支援教育の支援手法と障害者福祉の支援手法を取り入れた知的障害者の口腔保健行動を支援する包括的なアセスメントツールの開発を目指している。初年度は特別支援教育において実践されている知的障害者の実行機能の困難さに対する支援を参考に、障害者福祉施設通所の知的障害者を対象として視覚支援媒体を用いて歯科衛生士が歯磨き指導の予備的研究を行った。その結果、歯磨き指導の前後において口腔衛生状態の改善傾向と歯磨き支援量の減少傾向が認められた。その一方で、対象者の障害種別は知的障害のみの者や知的障害と発達障害などの他の障害を重複してもつ者と様々であり、障害種別の重複、精神年齢および障害の程度などによる歯磨き支援効果に着目して、その効果を分析した。社会生活年齢の判定にはSM社会生活能力検査を用いて、社会生活年齢を小学校入学年齢である7歳を基準として対象者を7歳未満と7歳以上に分け、歯磨き評価時（計6回）の口腔衛生状態と歯磨き指導の初回と最終回における歯磨き支援量を評価した。その結果、口腔衛生状態は歯磨き評価時（計6回）において社会生活年齢7歳以上群で改善傾向が認められ（p<0.068）、7歳未満群では改善は認められなかった。歯磨き支援量は、歯磨き指導の初回と最終回の比較において7歳以上群（p<0.144）と7歳未満群（p<0.180）のいずれも変化は認められなかった。社会生活年齢によって口腔衛生への効果が異なったことから、今後は、研究の参加人数を増やして障害種別の重複、精神年齢や障害の程度ごとに口腔保健行動の問題点を把握することや支援方法を検討することの必要性が確認できた。
