令和2年の厚生労働白書によると、2040年には、現役世代の急減と高齢者人口のピークが同時に訪れると推計されている。高血圧の有病率は加齢とともに上昇するが、我が国において就労中の世代でも高い値を示している。持続可能かつ質の高い保健医療を実現する一つの施策として、ポリファーマシーの源流である処方カスケードの発生防止を現役世代に有病率の高い疾患で検討する必要がある。しかし、処方カスケードに関する報告は少なく、大規模なレセプトデータ等を活用した分析は皆無である。本研究ではジヒドロピリジン系カルシウム拮抗薬（DH-CCB）による下肢浮腫に対するループ利尿薬の処方を例に、日本における処方カスケードの発生実態を可視化し、その医療費に与える影響を明らかにすることを目的とした。令和4年度では、研究デザインを固め、DeSC社のデータを入手し、デザインに従ったデータセットを作成した。データセットの作成には、医薬品、疾患、診療行為のマスタの整備、データセットの定義書を含む。また、機材を工夫し、プログラミングの環境を構築した。プログラミングには、名城大学と昭和大学のダブルコーディングを標準とした。DeSC社提供の4,328,134症例から、適格基準に従って、352,666症例が抽出された。
