项目摘要

抗がん剤使用が多種の循環器合併症を引き起こすことが報告されてきたが、新薬開発が続く中、単剤による未知の副作用、および複数併用による未知の増強作用が存在すると想定される。本研究は①抗がん薬の未知の循環器合併症、および複数の薬剤による未知の相互作用について、DPCデータベースを用いて発見する、②発見された抗がん剤の未知の副作用について、基礎医学的検証を行うことで、発現メカニズムを検証し、予防処置・薬剤の開発につなげる、以上の2点を目的としている。抗がん薬による循環器疾患の合併症として、まずは抗がん薬と心機能障害・心不全発症の関連について探索を進めた。担癌患者で心不全入院した患者の前入院での抗がん薬使用をレトロスペクティブに解析することで、使用されている頻度の多い薬剤のいくつかを見出した。上記にはアントラサイクリン系薬剤など既知の薬剤もあったが、それ以外でこれまであまり日本人では報告が少なかった薬剤も複数見出された。それらを候補薬剤として、薬剤の使用後心不全での入院の有無がどれくらいの割合で起こっているかを前向きに探索している。その中の一つとしてEpidermal growth factor receptor (EGFR) inhibitor であるosimertinibでは、2107例の投与例のうち、35名が心不全を発症していた。心不全発症群と非発症群で、年齢および腎機能障害が、心不全発症の有意なリスクファクターとなっていた。今後は、osimertinibの他の薬剤との心機能障害における相乗効果の可能性の検証や、ラットの心筋細胞でのメカニズム検証を行っていく予定である。

Anti-drug use causes a variety of circulatory complications, which are reported in reports, new drug developments, unknown side effects in single drug use, and unknown enhancement effects in multiple drug use. Epidermal growth factor receptor (EGFR) inhibitor was found in 2107 patients with osimertinib and 35 patients with cardiac insufficiency.
