高齢化・少子化・多死社会が進む中で、質の高い終末期ケアが喫緊の課題である。しかし、大切な人の死を経験することが少なくなった今日では、死にゆく際に生じうる身体的な変化（dying process）を学ぶことができる機会が乏しく、家族が患者の人生の最終段階時に生じる身体的な変化に不安が増強し、患者に対して何もできないと感じて悲しみを抱えたり、死別後にも生前に何もできなかったと後悔や罪悪感にさいなまれる場合もある。そこで、今年度は、Dying processを家族に説明している医療従事者へのインタビューを実施し、説明時の臨床判断の実態を明らかにする予定であった。しかし、インタビューを予定した日程で新型コロナウィルス感染が拡大し、インタビューが実施できなかったため、文献レビューにて臨床判断時に考慮する点を検討した。Dying processを説明する際に考慮する点として、大きく患者要因・家族要因・組織要因があることを明らかにした。具体的には、患者要因には、疾患の種類、病気の経過、在宅死の希望などが抽出された。家族要因としては、心配や不安の程度、在宅死の希望、疲労の程度、キーパーソンなどが明らかになった。組織要因としては、関係職種間の連携の程度、医療従事者の説明に対する不安の程度、Dying processを説明する資料の有無と種類などが抽出された。また、患者と家族の関係性、家族構成などのように、患者の要因と家族の要因に該当するものといった、重なりあう要因があることも明らかにした。
