项目摘要

本研究の目的は、統合失調症者の身体活動量の測定、健康意識と生活習慣に関する調査を通して、地域で生活する統合失調症者の身体状態・健康意識・生活習慣の特性を明らかにし、フィジカルマネジメントプログラムを開発することである。2022年度は、精神疾患患者の身体健康管理に関する文献レビューを行なった。統合失調症、うつ病、身体合併症、活動量、身体機能、生活習慣、健康管理、保健行動をキーワードとして、MEDLINE、医中誌、CINAHL、PsychoINFOを用いて2002～2022年の原著論文を検索し、最終抽出した46件を吟味した。横断研究が21件と最多であり、縦断研究12件、文献研究8件、介入研究は5件であった。精神科外来通院患者の肥満の有病率は高いこと、健常者に比し統合失調症群は腹囲とBMIの有意な増加、気分障害群ではeGFRの低下とCKD比率の有意な増加が認められたこと、身体活動量が患者の人生における目的意識の程度と関連すること、統合失調症患者の身体リスクは入院か外来かの治療環境の違いに影響されること、身体活動量の低下は実行力・就労意欲・活力の低下と相関していることがわかった。また、生活習慣プログラムによる身体活動量と代謝の健康改善、自己記録による体重維持・予防意識づけ・食行動の認知改善、健康教育プログラムによる歩数の増加、有酸素運動による精神症状の軽減および認知力と心肺機能の改善、ライフスタイル強化治療による代謝機能の改善など一定の効果があった。しかしながら、いずれも対象者数が少なく、アウトカム指標は多様であった。以上より、精神疾患患者の身体的健康の保持に有効な看護支援プログラムを考案する必要性があると考えた。
