本研究は、地域医療の形成過程を「患者」「医療提供者」双方の視点から明らかにし、現代における地域医療の在り方を提言することを目的としている。申請者らは2019年8月に岐阜県大垣市に伝来する小寺家文書より日本最古の入院診療明細書を発見し、同年12月には愛知県新城市の旧信玄病院が発行したレセプト原簿、会計原簿等を確認した。本年度は以下の内容で研究を進めた。①先行研究整理：近代医療関連の書籍、文献、画像の収集及び整理を行った。整理は「医療提供者側」「患者側」いずれの視点で論じているかによって分類した。②小寺家文書の再精査：小寺家文書の見直しを行い、『小寺家文書目録』衛生医療に分類されないものの中にも医療関連の資料がないかどうかの確認を行った。旧所有者小寺登氏の協力の元、検討を加えるに足る資料を得ることができた。書状（書付、領収証）などに医療関連の記述が残されているケースがみられるため、次年度も引き続き継続する予定である。新たな資料の解読は次年度に行うこととした。③資料の画像化及び内容の確認：画像化の進んでいない「小寺家日誌」についてスキャンを進めた。家族の受療行動、医療に対する考えなど、多くの示唆を与える重要な内容を確認することができた。信玄病院資料については、目録に沿って資料の確認を行い、画像化の準備を進め、次年度に画像化を進めることとした。④現地調査：資料収集地域に赴き、地域の方々にヒヤリングを実施した（一部遠隔会議システムを使用）。
