2022年度は、認知症の人への居宅介護支援の実施における「認知症の人の日常生活・社会生活における意思決定支援ガイドライン」に示された意思決定支援の手続きの実施状況等について質問紙を用いた調査を行った。調査票の作成にあたっては、都内の介護支援専門員、認知症ケア及びプログラム評価の学識経験者からなる検討委員会を組織した。調査は、都内の介護支援専門員を対象に、インターネットを活用して行った。調査は2023年5年1月27日～2月27日に実施した。ガイドラインに示された内容をもとに作成された意思決定支援の手続き等に関する項目について、「十分にできていると思う」～「不十分であると思う」の4件法で尋ねた。また、当該項目について回答した際に想定した利用者の状態像と、支援が困難と感じた具体的な状況について尋ねた。アクセスのあった227件のうち、未完了のサンプルを除いた165件を分析対象とした。意思決定支援の手続き等に関する項目では、「概ねできていると思う」に回答が集中する傾向がみられたが、「やや不十分であると思う」と回答したものが全体の20％を超えていた項目の回答傾向をみると、特に意思決定能力の評価や、その手続きに関する項目であった。また、困難状況としては、「独居のため、日中や夜間の客観的な様子がわからない（情報収集ができない）」「危険があるにもかかわらず料理や散歩など、本人の生活様式や生活行動をやめてもらうことができない」等の項目に60％以上の回答者がチェックしていた。介護支援専門員は概ねガイドラインに沿う形でケアマネジメントを展開していることが示唆された。一方、意思決定能力の評価やその手続きに関する項目については、やや不十分と感じている介護支援専門員がおり、その具体的な手続きやプロセスについて可視化し、研修棟を通じて標準化を図っていく必要性が示唆された。
