项目摘要

本研究の目的は、身体症状症に対して、うつと不安に対する診断横断的な認知行動療法である統一プロトコル（UP）の安全性と有効性を検証することである。初年度である今年度は、当初の計画通り身体症状症に対する認知行動療法のレビューを行った。具体的には、研究数、介入方法、UPとの共通点や差異について、身体症状症に対するCBTの無作為化比較試験17本のレビューをし、介入方法、週数、セッション数、介入技法の整理を行った。レビューの結果、介入方法としてUPには、身体症状症に対するCBTにおいて使用されてきた対人関係技法を除くすべての技法（心理教育、行動的技法、認知的技法、感情的技法、曝露、身体的技法、行動実験、マインドフルネス、宿題）が含まれていた。実施週数は12週未満が3本、12週以上が14本、セッション数が10回未満は9本、10回以上は8本であった。実施時間50分未満が3本、50分以上が14本であった。このことから、UPも身体症状症に対して応用可能であり、一定の効果が期待できるものと推察された。ただし、先行研究では実施回数10回以下が半数を占めていたのに対し、UPは平均16回と相対的に回数が多いため、臨床実践の際に生じる参加者側、実施側の時間的なコストなどについては今後も更なる検討が必要と考えられる。UPをベースにしたプログラムやマテリアル整備については、研究代表者の産前産後休暇と育児休業取得に伴い、研究を中断したため実施できていない。
