项目摘要

新型コロナウイルスの世界的流行によってオンラインビデオカウンセリング（VCP：Videoconferencing Psychotherapy）のニーズが急激に高まっている。しかし、VCPのドロップアウト（治療中断）率は非常に高く、２回目以降のカウンセリング継続を希望しない割合は7割にもおよぶ一方、中断理由の詳細は明らかになっていない。本科研研究の目的は，以下であった。1. VCPにおけるクライエントニーズと提供サービスの実態把握，2. VCPにおけるドロップアウトおよび治療効果を予測する要因の仮説構築，3. カウンセリングニーズの調整介入，治療関係へのコンサルテーション介入等を通して，適切な治療環境を構築し，ドロップアウト率および治療効果向上を目指す。2022年度は，１を実施するとともに２の仮説変数を抽出した。民間企業において実施されたVCPデータを用いて，利用実態の記述を行うとともに，1‐2回目のセッション間のドロップアウト/継続を予測する説明変数の探索を目的として階層的ロジスティック回帰分析を行った。その結果，セラピストの臨床経験年数が長いと継続しやすい傾向，クライエントがカウンセリングセッション終了時に「効果を実感」すると継続しにくく，カウンセリングに対する「効果の予感」を感じると継続しやすい傾向が示された。今後は，cl．の症状の程度，また治療効果を測定の指標に含めることで，カウンセリング継続の必要性が高いと推察されるcl．におけるドロップアウト要因の検討を行うとともに，「効果の予感」等，カウンセリングへの期待をより精緻に定量化し，介入操作を行う等の前向き研究が求められる。
