项目摘要

肝動脈化学塞栓療法（TACE）は動脈性に栄養される肝細胞癌に対して、経カテーテル的に腫瘍の栄養動脈に抗癌剤と塞栓物質を投与し、腫瘍を虚血・壊死に陥らせる方法である。肝細胞癌に対する優れた治療法であるが、治療を繰り返すことによる肝予備能の障害が問題点である。我々はこれを克服するため新たなTACEの手法であるBOAI-TACE（Transarterial Chemoembolization of Balloon-Occluded Alternate Infusions of Cisplatin and Gelatin Particles for Hepatocellular Carcinoma）を開発した。BOAI-TACEは、腫瘍を栄養する肝動脈にバルーンカテーテルを留置し、バルーン閉塞下にバルーンより末梢から、シスプラチン水溶液と破砕ゼラチン粒子を交互に動注する手法である。従来のTACEで使用されているリピオドールは使用しない。過去に我々は、本手法による多施設共同前向き試験を実施し、BOAI-TACEは従来のTACEと比較して1)治療効果は従来のTACEと比較して遜色ない、2)肝機能温存に優れる、ことを報告している。また、この研究結果を後ろ向きに解析したところ、多発あるいは巨大肝細胞癌に対する治療効果に優れる、ことが分かった。本研究では、これを前向き試験で証明するために、特定臨床研究として、「従来の肝動脈塞栓術では治療困難とされるup to 7 out肝細胞癌に対するシスプラチン溶液と破砕ジェルパートを用いたバルーン閉塞下動脈塞栓術の有効性試験」を行うこととした。現在では、すでに倫理審査委員会を承認を得ており、上記特定臨床研究の進行中である。

Hepatic arterial chemoembolization therapy (TACE) is a method of treating hepatocellular carcinoma by administering anti-cancer agents and thrombolytic substances to the vascular system.
