项目摘要

2022年度は、2011～19年の小児下痢症患者の便由来大腸菌3291株から143株のatypical EAEC (aEAEC)を決定し、病原遺伝子、基質特異性拡張型β-ラクタマーゼCTX-M遺伝子の検出、CTX-M型別を行った。まず3291株からaggR遺伝子保有EAEC（typical EAEC, tEAEC）81株とその他の下痢原性大腸菌の病原遺伝子を保有する株を除いた3099株を対象に、バイオフィルムアッセイでバイオフィルム形成能（BI）の高い株（＞OD0.2 ）193株を抽出し、HEp-2細胞付着試験でaEAEC 143株を決定した。aEAEC143株中19株（13.3％）が典型的な凝集付着（tAA）を示し、124株（86.7％）は非典型的な凝集付着（aAA）を示した。BIはtAA 0.75、aAA 0.4とtAAの方が高い値を示した。CTX-M遺伝子は9株（6.3％）が保有し、そのうち6株（66.7％）はaAAで、CTX-M型は14が3株、15が3株、27が2株、97が１株であった。K1莢膜遺伝子は33株（23.1％）が保有し、そのうち32株（97％）がaAAであった。PhylogroupはB2が69株（48.3％）と最も多く、68株（98.6％）がaAAであった。tEAECのプラスミドと染色体上の病原遺伝子保有株が49株（34.3％）、phylogroup A・B1・B2に属する多くの株がその両方の病原遺伝子を同時に保有していた。尿路病原性大腸菌の病原遺伝子はphylogroup EとUT(untypable)以外の株で検出された。CTX-M/K1遺伝子保有株がCTX-M型14と15の2株検出されたが、14の株はphylogroup B1に属しEAECの染色体とプラスミドの両方の病原遺伝子を保有し、付着はtAAを示した。tEAEC 81株についてもHEp-2細胞付着試験を行い、tAA 28株（34.6％）、aAA 24株（29.6％）がみられたが、29株（35.8％）はBIが0.2以下で凝集付着を示さない株であった。

From 2022 to 2011 - 19, there were 3291 strains of Escherichia coli and 143 strains of atypical EAEC (aEAEC) in patients with infantile diarrhea. Pathogenic vectors, matrix specific β-strains and CTX-M types were identified. 3291 strains were isolated and 81 strains remained EAEC (typical EAEC, tEAEC). Among them, 3099 strains were isolated and 193 strains were identified by HEp-2 cell test. Among 143 aEAEC strains, 19 (13.3%) showed typical agglutination (tAA) and 124 (86.7%) showed atypical agglutination (aAA). BI is tAA 0.75, aAA 0.4 and tAA is high. 9 CTX-M strains remained (6.3%), 6 (66.7%) aAA, 14 (3) CTX-M strains, 15 (3) CTX-M strains, 27 (2) CTX-M strains, 97 (1) CTX-M strains. 33 strains (23.1%) of K1 capsule were retained, and 32 strains (97%) were aAA. Phylogroup B2 69 (48.3%) 49 strains (34.3%) of tEAEC had the pathogen on chromosome, and most of the strains belonged to phylogroup A·B1·B2 had the pathogen on chromosome. Pathogenic vectors of uropathogenic E. coli were detected except for phylogroup E and UT (untypable) strains.
